I hereby:
Authorize the Pharmaceutical Society of Jamaica (hereafter called the PSJ) to accept and act on instructions issued by me via email from email address(es) in schedule, in respect of any and all requests for service, payments made to the PSJ, or any other form of correspondence between myself and the PSJ. Such correspondence with the PSJ may include:
1. Registering for services provided by the
PSJ;
2. Processing of Payments.
3. Requests for Information pertaining to any
of the PSJ's services.
4. Any other form of correspondence deemed
necessary for the PSJ to deliver its products
or services, that is not mentioned herein.
Authorize the PSJ to send updates via email to the email address(es) in the schedule in respect to any correspondence I may have with them.
Indemnify and hold the PSJ, its agents and employees harmless upon demand in respect of all claims, liabilities, losses, damages, costs and expenses whatsoever which may be incurred by or asserted against the PSJ, its agents and employees in connection with or arising directly or indirectly from any action taken in accordance with the instructions received by email from me.
The PSJ shall not be liable for acting on the instructions received which may not have been authorized by me and the instructions may have been misinterpreted or the PSJ may have made errors of omission or commission, or for any delay in compliance with the instructions.

Further, the PSJ reserves the right to reject any request without giving a reason for such rejection and shall not be required to verify any instructions received prior to taking steps to carry out the instructions given.
