PHARMACEUTICAL SOCIETY OF JAMAICA AWARDS 2016

The Pharmaceutical Society of Jamaica (PSJ) is pleased to announce and invite nominations for its awards 2016, the fourth annual PSJ Awards.

Nominate a pharmacist who’s gone above and beyond!

We know pharmacists are a humble bunch, so we need your help in recognizing some of the most hard-working, dedicated and innovative members of the profession.

Whether you’re a fellow pharmacist, patient, colleague or community member, there are opportunities to submit a nomination for the 2016 PSJ Awards!

These annual awards celebrate pharmacy excellence in innovation, mentorship, community service and more.

Recipients will be honoured at a special Awards Banquet; held during pharmacy week at the Annual inauguration ceremony for newly registered pharmacists.

Pharmacists serve as: counselors, educators, advisors, managers, dispensers, caregivers, mentors, decision maker and researcher. They go above and beyond to provide clinical services.

Awards and Criteria

1. JAMAICAN PHARMACIST PRIMUS (JPP)

Sponsored by: Lasco Distributors

An award to honour a pharmacist who has had distinctive, transforming and lasting influence on the profession of Pharmacy in the Jamaica. Only one (1) Jamaican pharmacist will be named to the Honour Roll annually. The said Honouree will enjoy the right to use and be addressed by the honorary title Jamaican Pharmacist Primus (JPP)

The Winner of this award will:-

  • Be sashed Jamaican Pharmacist Primus
  • Receive a Plaque/Trophy with the PSJ logo entrenched
  • Have his /her name entered in the PSJ Roll of Honour

Other incentives include:-

  • -1 night accommodation at the succeeding PSJ Annual Conference
  • A cash award ( $10,000.00)

Any member of the Pharmaceutical Society of Jamaica may make a nomination

The nomination must be supported by at least two members of the PSJ.

The nominee must:-

  • Be a registered pharmacist with no less than fifteen (15) years experience
  • Be a member of the Pharmaceutical Society of Jamaica for no less than ten years
  • Have attended no less than five PSJ Retreats/Conferences
  • Have a consistent record of attendance or participation in Continuing Education programmes
  • Have an exceptional record of accomplishment in community practice and/or academia and/or research and/or professional leadership organization and/or manufacturing and any other aspect of pharmacy practice
  • Have made extensive contribution to pharmacy that has transformed pharmacy practice, enriched professional value with national impact
  • Have publications to his/her credit or have engaged in and published original research
  • Have mentored or assisted pharmacy students
  • Have a flawless ethical and legal record.

Rules:-

All persons making nominations must:-

  1. Complete the prescribed nomination form and inform the nominee for approval
  2. Submit form to the Pharmaceutical Society of Jamaica no later than Friday November 4, 2016
  3. The decision will be made by the Executive Committee of the PSJ
  4. No more than one awardee will be named annually
  5. The winner will be announced during the Annual Awards Banquet
  6. The decision of the Executive Committee shall be final

2. JAMAICAN PHARMACIST OF THE YEAR

Massy Distributors

An award for outstanding contribution to the Profession of Pharmacy, Community and the Pharmaceutical Society of Jamaica.

The Winner of this award will:-

  • Be sashed Jamaican Pharmacist of The Year
  • Receive a Plaque/Trophy with the PSJ logo entrenched
  • Have his /her name entered in the PSJ Roll of Honour

Other incentives include:-

  • 1 night- accommodation at the succeeding PSJ Annual Conference
    Waived membership fee for 2 years

Any member of the Pharmaceutical Society of Jamaica may make a nomination.

The nominee must:-

  • Be a Registered Pharmacist in Jamaica
  • Be a member of the Pharmaceutical Society of Jamaica
  • Have a good record of participation in Continuing Education
  • Have made outstanding contribution to Pharmacy either by the introduction of new concepts or ideas, products or techniques that have benefitted healthcare over the past year.
  • Has earned the respect of professional colleagues
  • Have made outstanding contribution to their community
  • Have a flawless ethical and legal record.

Rules:-

All persons making nominations must

  1. Complete the prescribed nomination form and inform the nominee for their endorsement
  2. Submit form to the Pharmaceutical Society of Jamaica
  3. The forms must be submitted to the PSJ on or before Friday November 4, 2016
  4. The decision will be made by the Executive Committee of the Pharmaceutical Society of Jamaica
  5. The Executive Committee of the Pharmaceutical Society of Jamaica reserves the right to make a nomination
  6. The announcement of the winner will be made during the Annual Awards Banquette
  7. The decision of the Executive Committee of the Pharmaceutical Society of Jamaica shall be final.

3. THE JAMAICAN PHARMACY YOUTH LEADER AWARD

SPONSORED BY EVERYBODY’S PHARMACY

An Award for a young pharmacist who has demonstrated leadership during the training and/or early years of professional life.

The Winner of this award will:-

  • Be sashed Jamaican pharmacy youth leader of the Year
  • Receive a Plaque/Trophy with the PSJ logo entrenched
  • Have his /her name entered in the PSJ Roll of Honour

Other incentives include:-

  • The winner of this award will receive free membership dues for one year

Any member of the Pharmaceutical Society of Jamaica may make a nomination

The nominee must:-

  • Be a registered pharmacist with the Pharmacy Council of Jamaica
  • Be a member of the Pharmaceutical Society of Jamaica
  • Have displayed interest in professional organizations or student groups during training
  • Display best practice in the carrying out of professional duties
  • Have a good reputation in professional circles

Rules:-

All persons making nominations must:-

  1. Complete the prescribed nomination form and inform the nominee for endorsement
  2. Submit form to the Pharmaceutical Society of Jamaica
  3. The form must be submitted to the Pharmaceutical Society of Jamaica no later than Friday, November 4, 2016
  4. The pharmacy training institution may be consulted
  5. The decision will be made by the Executive Committee of the Pharmaceutical Society of Jamaica
  6. The Executive of the Pharmaceutical Society of Jamaica reserves the right to make a nomination
  7. The announcement of the winner will be made during the Annual Awards Banquette
  8. The decision of the Executive of the Pharmaceutical Society of Jamaica shall be final

4. THE JANETTE STEWART FAIRPLAY AWARD

Sponsored by: PSJ

An Award to a member of the Pharmaceutical Society of Jamaica who has displayed conduct that conforms to the highest ethical and humanitarian standards.

The Winner of this award will:-

  • Receive a Plaque/Trophy with the Pharmaceutical Society of Jamaica logo entrenched
  • Have his /her name entered in the Pharmaceutical Society of Jamaica Roll of Honour

Other incentives include:-

  • The winner of this award will receive free membership dues for one year

Any member of the Pharmaceutical Society of Jamaica may make a nomination.

The nominee must:-

  • Be a Registered Pharmacist and have displayed conduct of the highest ethical and humanitarian standards.

  2. Submit form to the Pharmaceutical Society of Jamaica

5. THE JAMAICAN DISTINGUISHED PHARMACY RESEARCHER AWARD

SPONSORED BY: Eden Gardens

An Award to an individual, organization or institution that engages in pharmacy research and encourages the development and effective use of pharmacy research among practicing pharmacists.

The Winner of this award will:-

  • The Jamaican Researcher Plaque/Trophy with the Pharmaceutical Society of Jamaica logo entrenched
  • Have his /her name and/or institution entered in the Pharmaceutical Society of Jamaica Roll of Honour

Criteria:-

  • Encourages the development and effective use of pharmacy research among practicing Pharmacists.
  • Disseminates research results through presentations and or publications
  • Receives funded support from local, regional and international agencies.

Any member of the Pharmaceutical Society of Jamaica may make a nomination

Rules:-

All persons making nominations must:-

  1. Complete the prescribed nomination form and inform the nominee for their endorsement
  2. Submit form to the Pharmaceutical Society of Jamaica
  3. The form must be submitted to the Pharmaceutical Society of Jamaica no later than Friday, November 4, 2016
  4. The decision will be made by the Executive Committee of the Pharmaceutical Society of Jamaica
  5. The announcement of the winner will be made during the Annual Awards Banquette
  6. The decision of the Executive Committee Pharmaceutical Society of Jamaica shall be final.

6. THE JAMAICAN PHARMACY BUSINESS AWARD

SPONSORED BY: Dr. NORMAN DUNN

An award for the Pharmacy that epitomizes the best of the pharmacy business culture. This centre of excellence must provide the setting, services and support for community in keeping with the soaring goals of pharmacy.

The Winner of this award will:-

  • Receive the Jamaican Pharmacy Plaque/Trophy with the Pharmaceutical Society of Jamaica logo entrenched
  • Have his /her name and/or institution entered in the Pharmaceutical Society of Jamaica Roll of Honour

Other incentives include:-

  • Free advertisement on the PSJ Website for one year
  • One free ad in the retreat magazine the following year
  • Permission to use the accolade in its advertising and promotion for one year

Any member of the Pharmaceutical Society of Jamaica may make a nomination

The pharmacy must:-

  • Be registered and employ the full time services of a registered pharmacist
  • Be in good standing with the Pharmacy Council of Jamaica
  • Dedicate no less than 10% of its floor space to designated pharmacy services
  • Display patient centered elements in the layout including a counseling area
  • Not sell tobacco related products or alcoholic beverages
  • Support Community based activities

Rules:-

All persons making nominations must:-

  1. Complete the prescribed nomination form and inform the nominee for endorsement
  2. Submit form to the Pharmaceutical Society of Jamaica
  3. The form must be submitted to the Pharmaceutical Society of Jamaica no later than Friday, November 4, 2016
  4. The decision will be made by the Executive Committee of the Pharmaceutical Society of Jamaica.
  5. The Executive Committee of the Pharmaceutical Society of Jamaica reserves the right to make a nomination
  6. The announcement of the winner will be made during the Annual Awards Banquette.
  7. The decision of the Executive Committee of the Pharmaceutical Society of Jamaica shall be final
  8. Photographs showing the front and interior of the pharmacy would be useful.

7. THE JAMAICAN PHARMACEUTICAL INDUSTRY AWARD

SPONSORED BY: SUPERMED PHARMACY

An Award to an Industry Partner (DISTRIBUTIVE OR MANUFACTORING PHARMACY/COMPANY) who has shown outstanding continued service and support to Pharmacy and the betterment of health care.

The Winner of this award will:-

  • Receive the Jamaican Pharmaceutical industry Plaque/Trophy with the PSJ logo entrenched
  • Have his /her name and/or institution entered in the Pharmaceutical society of Jamaica Roll of Honour

Other incentives include:-

  • One free ad in the retreat magazine the following year
  • Permission to use the accolade in its advertising and promotion for one year

Criteria:-

  • A partner who has contributed significantly to the Jamaican Pharmacy industry for three (3) or more consecutive years plus in the year of the award.

Any member of the Pharmaceutical Society of Jamaica may make a nomination

Rules:-

All persons making nominations must:-

  1. Complete the prescribed nomination form and inform the nominee for endorsement
  2. Submit form to the Pharmaceutical Society of Jamaica
  3. The form must be submitted to the Pharmaceutical Society of Jamaica no later than Friday, November 4, 2016
  4. The decision will be made by the Executive Committee of the Pharmaceutical Society of Jamaica
  5. The announcement of the winner will be made during the Annual Awards Banquette
  6. The decision of the Executive Committee of the Pharmaceutical Society of Jamaica shall be final.

8. THE JAMAICAN LIFETIME ACHIEVEMENT IN PHARMACY AWARD

SPONSORED BY: PSJ

An Award that recognises a Pharmacist whose career has significantly affected Pharmacy Health Care and Communities and who has practiced pharmacy for more than 30 years.

The Winner of this award will:-

  • Be sash Jamaican Lifetime Achievement in Pharmacy
  • Receive a Plaque/Trophy with the Pharmaceutical Society of Jamaica logo entrenched
  • Have his /her name entered in the Pharmaceutical Society of Jamaica Roll of Honour

Other incentives include:-

  • 1 night accommodation at the succeeding Pharmaceutical Society of Jamaica Annual Conference
  • Free entry into all PSJ’s CE’s for one year

Criteria:-

  • Has demonstrated significant achievements in improving pharmacy health care.
  • Has demonstrated resourcefulness and creative approaches to implement change.
  • Has demonstrated a proactive response to health care needs.

  2. Submit form to the Pharmaceutical Society of Jamaica
  3. The form must be submitted to the Pharmaceutical Society of Jamaica no Friday, November 4, 2016
  4. The Pharmacy Council of Jamaica may be consulted for verification
  5. The decision will be made by the Executive Committee of the Pharmaceutical Society of Jamaica
  6. The Executive Committee of the Pharmaceutical Society of Jamaica reserves the right to make a nomination
  7. The announcement of the winner will be made during the Annual Awards Banquette
  8. The decision of the Executive Committee of the Pharmaceutical Society of Jamaica shall be final.

9. THE PHARMACEUTICAL SOCIETY OF JAMAICA GRATITUDE AWARD(S)

SPONSORED BY: PSJ

An Award that recognises all members of the current executive of the Pharmaceutical Society of Jamaica and the most outstanding committee member from each committee who has demonstrated and given yeoman’s service to the society in the current year.

The Winners of this award will:-

  • Receive a Plaque/Trophy with: the Pharmaceutical Society of Jamaica logo entrenched, service beyond the call of duty/outstanding contribution to the development of pharmacy/outstanding service as a committee member
  • Have his /her name entered in the PSJ Roll of Honour

Criteria:-

  • Must be an executive or committee member of the PSJ
  • Have given voluntary service to the society in the current year

All executive members of the Pharmaceutical Society of Jamaica are eligible for this award

Rules:-

  1. The decision will be made by the Executive Committee of the Pharmaceutical Society of Jamaica
  2. The committee chairperson and the executive of the Pharmaceutical Society of Jamaica reserve the right to make nominations for those committee members who will be awarded.
  3. The awards will be made during the Annual Awards Banquette
  4. The decision of the Executive Committee of the Pharmaceutical Society of Jamaica shall be final.

All persons who have accepted nominations must arrange to be present during the Awards Ceremony, Tuesday November 22, 2016 during Pharmacy Week, November 20 – 26 2016, in Kingston or inform the PSJ of the name of their representative.
